What a useful legal intake record should contain

A transcript tells you what was said. An intake record tells your team what to do. The difference matters when several people touch a new inquiry, the caller switches languages or the first transfer fails.

Identity and safe contact

Start with who is calling, whether they are calling for themself, the safest callback number, preferred language and whether voicemail or text is safe. Contact permission should be recorded by channel. A phone number is not blanket permission to text, email or use WhatsApp.

Caller-stated facts

For Personal Injury, the record usually needs the caller’s description of what happened, when and where it happened, parties or organizations named, injuries described, treatment received or planned, reports and available evidence. Preserve uncertainty. ‘Caller thinks it happened in late May’ is more useful and honest than an invented exact date.

Conflict-safe names

The system may collect names the firm needs for later review. It should not mark the matter conflict-free. Conflict analysis belongs to the firm and often depends on records and professional judgment beyond the first call.

Operations, not legal scoring

A record should show why the team needs to act: transfer requested, catastrophic fact reported, appointment booked, missing information, caller already represented, or special caller type. That is an operational priority reason. It is not a conclusion that a case is valuable or should be accepted.

Provenance and trust

A reliable record also stores the source call or session, timestamps, workflow version, language, consent, transfer attempts, tool results and delivery status. This lets the firm understand what happened and lets the provider investigate a mistake.

A simple quality test

Give the record to a team member who did not hear the call. Can they see what the caller stated, what remains unknown, what the system did and what they must do next? If they have to read the entire transcript to answer those questions, the record is not finished.
